miscellaneous Sample Functional Assessment and Behavior Support Plan Number 1 This is a Functional Behavior Assessment and Behavior Support Plan Report Michael Fabrizio and Sara Pahl wrote for a six year old boy with severe autism. The FBA assesses hitting others and screaming at both home and in school. It includes approved use of restrictive techniques (i.e., restraint, physical escorting, and geographic isolation). While I certainly do not recommend that such techniques be employed without due precautions, I thought seeing how these may be responsibly included within a behavior support plan might be helpful. Sample Functional Assessment and Behavior Support Plan Number 2 This is a Functional Behavior Assessment and Behavior Support Plan Report Michael Fabrizio and Sara Pahl wrote for a six year old boy with multiple mild disabilities (e.g., ADD, ODD). The FBA assesses inappropriate toileting and inappropriate attention shifts during group instruction. It includes embedded graphed results from the Functional Assessment Observation Form as well as sample graphs for the child's IEP team to use when measuring variables important to the effectiveness and implementation of the plan. Sample Functional Analysis and Behavior Support Plan This report describes the results of a functional analysis (an experimental analysis) of the factors influencing the misbehavior of a fifth grade student with Cri du Chat syndrome. The report details how and why we moved from functional assessment (descriptive analysis) to functional analysis (experimental analysis) and also includes the behavior support plan we recommended for this student. A note about the above plans: I do not expect that the FBA and BSP report you complete for this class will be as extensive as either of the samples I have provided you with here. They are only provided to you as examples in the hope that seeing how finished product might look will help you in completing your assignments for this class.
